We are providing this notice to alert you of an important change regarding the distribution of Form 1095-C. Champion Homes is no longer required to automatically mail the Form 1095-C to individuals.
The forms are available electronically in UKG (formerly UltiPro). You can get there by following the path below:
Menu > Myself > Benefits > 1095-C
Electronic access to 2024 forms will be perpetual as long as you have an active account in UKG. Terminated employees may request a copy from their local Champion HR rep. We may provide the copy to you with your consent.
